Consent Form to Administer Prescription Tablets or Medicine

Antibiotics

Sometimes, children need to be on antibiotics if they are fighting off an illness. We have trained first aiders who can administer medication. To do this, please complete the above form and hand the antibiotics to the School Office.

Inhalers

If your child requires an inhaler please complete the above form, and hand the inhaler to the teacher. Inhalers are kept in the classroom with the child.

*Please note, we can only administer medication which has been prescribed by a Doctor.
